Services for Seniors and Caregivers

HELP – You need PSCS now

This package helps the family who has a loved one in the hospital or other critical state, and realizes that services need to be arranged immediately (within a week or two). The family may need to move the individual to a community or may ask that their loved one be taken home.

Family Steps

This package is ideal for the individual who wants to leave directions and wishes for his/her loved ones after he/she passes away. A portfolio will be created for the family, which will assist them in meeting the objectives of their loved one after he/she passes away.

Understanding My Options

This package applies to the individual who is uncertain of what his/her options are.  A customized portfolio is created for the individual, which addresses his/her short term and long term needs and desires.

• 1 Hour Consultation
• Utilize necessary resources (sources of income, benefits, etc.)
• Prepare a First Steps Portfolio – a customized portfolio that describes what options are most feasible for the individual
• Recommend necessary professionals
• Remain in constant communication with all parties

Staying In

This package is for the individual that is determined to stay in his/her home as long as possible. PSCS will assist the individual and/or family members identify and obtain the resources that are most desired and affordable. This package includes all of the services included in 'Understanding My Options' and additional items listed below.

• Arrange services as necessary (care, transportation, groceries, housekeeping, etc.)
• Accompany family/client on tours and other appointments
• Fill out paperwork as needed (applications, etc.)
• Ensure essential documents are in order and received by the appropriate parties (legal documents, medical evaluation, etc.)
• Review what has been done and what needs to be done
• Follow up with the individual 2 times to assure they are satisfied with the services they are receiving

Moving Out

This package assists the individual or family who is looking to move out of their home into a retirement community. This can be an overwhelming process, but PSCS is here to walk beside you every step of the way. This package includes all of the services included in 'Understanding My Options' and 'Staying In' and additional items listed below.

• First Steps Portfolio [3-5 communities selected (additional communities can be included)]
• Arrange outside services and/or recommend professionals who will assist the family with moving items, selling unwanted items, selling home, etc.
• Provide a checklist of "What to Bring When Moving"
• Accompany the family and individual the day of transition
